phone keeps switching between handset and headset

Has this happened to anyone else? While I’m using the handset, the phone keeps switching over to headset, though I don’t have a headset plugged in. This causes the phone call to go in-and-out.
Anyone experience this? I’ve tried multiple OS — (5 and 6) — still not working.
